You open this dialog box by attempting to edit the read-only Simulation Waveforms section (Simulator Report Window).

Allows you to open the editable input vector source file, and gives you the option to overwrite the input vector source file with the results of the simulation. To avoid receiving this dialog box in the future, you can open the input vector source file with the Open command.
